Your teams are providing more intensive care. Has your care assessment been updated to reflect this?

ZZP Reassessment monitors your ECD daily and flags when a client’s care needs increase. It is based on the CIZ’s steps, so your team leader can immediately see where a reassessment may be necessary—without requiring any additional documentation from your care providers.

ZZP Reassessment was developed and tested in collaboration with Esculine, a division of SureSync.

Self-Employed Reassessment

Automatic detection instead of manual searching

Teams sometimes provide VV7 or VV8 care for months on end, even though the care assessment is still listed as VV5 or VV6. This rarely gets noticed right away: team leaders and care managers are busy with day-to-day care, and the warning signs get lost among the reports.

ZZP Reassessment monitors your ECD daily and identifies cases where the level of care is increasing. For each client, you receive a substantiated alert, structured according to the steps used by the CIZ: problem behavior, interventions, effect, and prognosis. This allows your team leader to quickly assess whether a reassessment is warranted.

On average, patients are notified of a reassessment three months earlier

In an analysis of ECD data, the model identified the need for a reassessment, on average, three months earlier for a portion of the clients. In the case of a reclassification to a higher care profile, this represents approximately €5,000 in reimbursement per client that you would otherwise miss out on—without any additional documentation burden for your care providers.

Objective CIZ assessment, not a black box

The AI analyzes reports based on the steps the CIZ follows during reassessment: problem behavior, interventions, impact, and prognosis. This allows you to see where evidence is still missing from the file. CIZ applications are never submitted automatically: control over every decision remains entirely with the team leader and care manager.

Processed securely, directly linked to your ECD

ZZP Herindicatie runs entirely on Dutch servers and never sends patient data to Big Tech or parties outside the Netherlands. The AI securely analyzes daily reports in commonly used electronic patient records (EPRs) such as Ons, PUUR, and Ysis.

SureSync ZZP Reassessment is a dashboard that monitors your ECD reports on a daily basis and flags instances where the care provided exceeds the current ZZP assessment level. This allows you to quickly identify which clients require a reassessment.

The module was developed for inpatient elder care, such as nursing homes with more than 20 residents. The first version was developed in collaboration with the Laurens care facility and validated in practice.

The AI evaluates reports based on the four official CIZ pillars—problem behavior, interventions, impact, and prognosis—and clearly shows exactly where evidence is still missing from the file. This allows you to build a well-documented file before submitting an application.

No. The AI never automatically submits a CIZ application. Control remains entirely with the team leader and care manager; the tool provides support in the form of indicators and supporting evidence, but the assessment and the application itself remain the responsibility of human staff.

Reassessments are identified and submitted, on average, 3 months faster. A reassessment from VV5 to VV7 can quickly result in additional reimbursement of approximately €5,000 per client, in addition to reducing the team’s workload because signs are no longer overlooked.

Yes. The module operates in full compliance with the GDPR on servers located in the Netherlands. Patient data is never sent to Big Tech or parties outside the Netherlands; the AI processing is applied to your organization’s data, not the other way around.

SureSync ZZP Reassessment was developed in collaboration with Esculine and validated in partnership with the Laurens healthcare organization, with the goal of identifying imbalances in care intensity at an early stage without placing an additional administrative burden on healthcare providers.
